GRANT NARRATIVE & ANNUAL VALUATION REPORT
The Final Report for the 2024-25 Grant Cycle is due Tuesday, July 15. All FAFCC members are also asked to submit the Annual Valuation Report, which collects patient services data from July 1, 2024 through June 30, 2025. A new version of the report has been integrated within the Salesforce Grant Portal for a more streamlined experience.
The valuation report plays an integral role in demonstrating the impact of our sector as well as the return on investment of state dollars through the State Funds Grant Program.
